I. ASSUMPTION OF RISK AND RELEASE OF LIABILITY
I, the undersigned, acknowledge and understand that participating in the TLC Futsal Classic, organized by Son of a Pitch Football Company (the "Event"), involves physical activity and inherent risks, including but not limited to falls, collisions, physical exertion, and possible injuries to myself and others. I fully assume these risks and agree to participate in the Event voluntarily.
In consideration of being permitted to participate in the Event, I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Son of a Pitch Football Company, the Event organizers, the College of Staten Island, their officers, directors, agents, employees, volunteers, sponsors, and any other persons or entities associated with the Event from any and all claims, demands, actions, or causes of action arising from personal injury, death, or property damage that may occur during the Event or while traveling to and from the Event, whether caused by the negligence of the released parties or otherwise.
II. HEALTH & SAFETY ACKNOWLEDGEMENT
I confirm that I am in good physical health and have no medical conditions that would prevent me from participating in the Event. I understand that it is my responsibility to consult a physician prior to participating if I have any concerns about my fitness level or health.
I agree to follow all health and safety guidelines set forth by the Event organizers, including but not limited to:
Wearing appropriate athletic gear, including proper footwear and shin guards (if required).
Following all instructions from event officials, referees, and staff during the Event.
Not participating if I am under the influence of alcohol, drugs, or any other substance that impairs my ability to play safely.
Immediately notifying event organizers or medical staff in case of injury or medical issue during the Event.
In the event of injury or medical emergency, I consent to receive medical treatment or emergency care as deemed necessary by qualified personnel. I understand that any medical expenses incurred are my responsibility.
III. COVID-19 AND INFECTIOUS DISEASE ACKNOWLEDGEMENT
I acknowledge the ongoing risks related to COVID-19 and other infectious diseases. I agree to comply with any COVID-19-related health protocols in place for the Event, including but not limited to wearing masks, social distancing, and undergoing temperature checks or testing if required.
I understand that despite all safety measures taken, there is an inherent risk of exposure to COVID-19 or other infectious diseases, and I voluntarily assume this risk by participating in the Event.
IV. MEDIA RELEASE
I grant permission to Son of a Pitch Football Company and their authorized agents to use photographs, videos, or other media taken during the Event for promotional purposes, including but not limited to websites, social media, and advertising materials, without compensation or further approval.
V. CONSENT AND AGREEMENT
By clicking the checkbox below and entering my name, I acknowledge that I have read and understood this waiver in its entirety and voluntarily agree to its terms. I understand that by accepting this waiver online, it is legally binding and will remain in effect for the duration of the Event. I also acknowledge that this waiver may not be revoked once accepted.
